Understanding Family Medical 离开

Family Care and Medical 离开 (FML) is available to eligible employees in accordance with the Federal Family and Medical 离开 Act (FMLA), the California Family Rights Act (CFRA) and Collective Bargaining Unit Agreements. FML may be used for yourself or an eligible family member.

Upon completion of FML, you are entitled to return to the same or similar position with equivalent benefits, pay and conditions of employment. You are also entitled to retain any health insurance benefits during time away from work due to FML.

Process to Request to Use Family Medical 离开

• 完成 Certification of Qualifying Exigency for Military Family 休假(FML) [pdf] and forward to the 离开 Program Manager along with the Certification of Serious Injury or Illness of Covered Service Member Military Family 休假(FML) [pdf].

• For yourself or an eligible family member:
  • 完成 离开 of Absence Request Form [pdf] and forward to the 离开 Program Manager along with the Medical Certification for Family Medical 离开 [pdf]. 
  • For a qualifying exigency for military family leave:

Process to Return to Work

完成 Return to Work Certification [pdf] and forward to the 离开 Program Manager. You may not return to work without the approval of the 离开 Program Manager.
